New State Mandated Immunization/Health Requirements

The Kentucky Cabinet for Health and Family Services and the Department for Public Health has released new immunization requirements for all students attending public school in Kentucky. 

Beginning July 1, 2018:

  • ALL students in grades K-12 must show proof of having received two doses of Hepatitis A vaccine
  • Students age 16 and older must show proof having received two doses on Meningococcal ACWY vaccine (also called MenACWY or Meningitis Booster) to attend school.

Listed below is the new immunization requirements as well as other health documents that are required for all public school students for 2018-2019 school year.  These medical documents should be on file at your child’s school. 

Kindergarten up to age 10 Medical Documents Required

  •  Physical- Initial completed between the dates September 2017- August 2018
  •  Eye Exam- Initial due by Dec 31, 2018
  •  New and Up To Date Immunization Certificate including the Hepatitis A-2 shot series
  •  Dental Screening/Exam- Completed between Ages 5-6 due by Dec. 31
  •  Birth Certificate

Age 11 till Graduation Medical Documents Required

 Physical- completed between the dates September 2017- August 2018; Completed within one year before entry into 6th grade or entry to a Kentucky school after sixth grade

 New and Up To Date Immunization Certificate including the Hepatitis A-2 shot series

  • 11 years- □Hepatitis A series  □Tdap  □Meningitis
  • 16 years- □Meningitis Booster

Kindergarten Registration Health Requirement

2018-2019 School Year

 

Required documents for Kindergarten registration are the following:

 

  1. Certified Birth Certificate (application attached)

 

  1. Preventative Health Care Examination Form- Initial Entry (see attached) *For first time enrollment into public school
    • Completion Date between August 10,2017 to present

 

  1. KY Eye Examination Form for School Entry (see attached)
  • Deadline: January 1, 2019

 

  1. KY Dental Screening/Examination Form for School Entry (see attached)
  • Must be 5 or 6 years old
  • Deadline: January 1, 2019

 

  1. Up-to-date Certificate of Immunizations (see attached)
    • 2 different types of Immunization Certificate
      • Standard Certificate with the following (4-5yr)
        1. 3  Hep B
        2. 5 DTap
        3. 4 Polio
        4. 3-4 Hib
        5. 2 MMR
        6. 2 Varicella
        7. 2 Hep A
      • Certificate of Religious Declination of required immunizations (see attached)

 

  1. (If Applicable) ALL prescribed or over the counter medication requires a School Medication form EVERY year. ALL medication must be checked in by the front office. (Blank form attached)

  1. Parents must complete a Laurel County School Health Consent Form each year and return to school before a child can be treated by the licensed school nurse.

CPR Certification Courses are offered monthly by the school nurses.  You must register in advance to participate. 

Upcoming scheduled CPR classes for LCBOE coaches and faculty are listed below.

Coaches need to check their CPR card expiration date and make a copy for their Athletic Director.

Participants MUST register for the classes.

 

Fall 2018 Schedule:

5:30 pm to 8:00 pm

Certification Course (2- 2.5 hours)

All CPR classes will also be held at the GC Garland Building 2nd floor.
