New Immunization/Health Requirements

New State Mandated Immunization/Health Requirements

The Kentucky Cabinet for Health and Family Services and the Department for Public Health has released new immunization requirements for all students attending public school in Kentucky. 

Beginning July 1, 2018:

  • ALL students in grades K-12 must show proof of having received two doses of  Hepatitis A vaccine
  • Students age 16 and older must show proof having received two doses on Meningococcal ACWY vaccine (also called MenACWY or Meningitis Booster) to attend school.

Listed below is the new immunization requirements as well as other health documents that are required for all public school students for 2018-2019 school year.  These medical documents should be on file at your child’s school. 

Kindergarten up to age 10 Medical Documents Required

  •   Physical- Initial completed between the dates September 2017- August 2018
  •   Eye Exam- Initial due by Dec 31, 2018
  •   New and Up To Date Immunization Certificate including the Hepatitis A-2 shot series
  •   Dental Screening/Exam- Completed between Ages 5-6 due by Dec. 31
  •   Birth Certificate

Age 11 till Graduation Medical Documents Required

  •   Physical- completed between the dates September 2017- August 2018; Completed within one year before entry into 6th grade or    entry to a Kentucky school after sixth grade
  •  New and Up To Date Immunization Certificate including the Hepatitis A-2 shot series

 

  • 11 years-  ____  Hepatitis A series  ____  Tdap  ____ Meningitis

 

  • 16 years- ____  Meningitis Booster
