This section allows students to record awards and recognitions received. Students may enter any awards/recognitions received.
As students receive new awards, they are encouraged to add the award to their ILP as soon as possible so the information is not forgotten.
When including awards, students should answer the following questions:
1. Name of award
2. Month/Year award was received
3. Who? What? Why? When? Where?
NOTE TO PARENTS:
Sixth grade students will receive an "Awards and Recognitions" page to complete prior to completing this section in the ILP. This page will help the student compile a list of awards/recognitions received, including the name of the award, date received, and why the award was received, as well as when and where the award was received.
Your child is asked to fill out the paper. However, sometimes students do not remember the information, especially if there have been several awards received. Your child is allowed to ask for your help to complete this page in order to gather the information he or she will need to enter in the ILP.
If your child asks for your help, please assist your child by providing the information he/she needs to complete the form.
The "Awards and Recognitions" paper needs to be returned on or before the due date. If this paper is not turned in on time, it will delay the process of your child completing the ILP. Please make sure your child has this paper ready to turn in by the due date or before.
